The MED3/eMED3 Fit Note policy has now been ratified and updated.
The Med3/eMed3 Fit Note, sometimes referred to as ‘The Statement of Fitness for Work or ‘sick note’, is a form used by authorised healthcare professionals to give advice on a patient’s fitness for work.
At our Trust, the following professions are legally permitted to issue a fit note:
- Doctors
- Nurses
- Midwives
- Pharmacists
- Occupational Therapists
- Physiotherapists
- Advanced Practitioners – with one of the above professional registrations
Actions for clinicians:
If you are a clinician that currently issues a fit note to patients in secondary care, then you must do the following actions as soon as possible but no later than Friday 13th December 2024.
- Read the Fit Note Policy here.
- Complete the eLearning for Health (elfh) ‘Fit Note’ mandatory training NHSE elfh Hub (e-lfh.org.uk)
- Request for your line manager to submit a SARC form to IT to ensure that you retain your access to the electronic form eMED3 form in Sunrise.
Please ensure that you complete these tasks as soon as possible. Failure to comply with the policy will have a detrimental impact on patients and add pressure to colleagues.
If you have already completed this training prior to this communication, please present your certificate to your line manager, you do not need to repeat it.
Actions for line managers:
It is your responsibility to:
- Read the Fit Note Policy
- Check that your clinician is eligible to complete a fit note
- Check that it’s within their role remit and update their job description (where applicable)
- Review the elfh Fit Note training certificate prior to completing a SARC form via Marval to request access to the fit note.

FAQ’s
I have completed the eLearning for health (elfh) Fit Note training already, do I need to complete this again?
No, you only need to complete the elfh Fit Note training once and show your certificate to your line manager who will need to ensure you retain access.
My profession is listed as legally permitted to issue a fit note; however, I do not currently issue fit notes, do I need to do anything?
No, you do not need to complete the actions unless it is within your current role remit.
I work in the place-based division how does the policy impact me?
If you work in the place division you do not need to take any action at this time, however we would strongly encourage you to complete the elfh Fit Note training.
Is the Sunrise training the same as the eLearning for health Fit Note training?
No, the Sunrise training/ quick reference guides illustrate how to issue a fit note using the Sunrise system.
Why do I need to complete training, I already know how to issue fit notes to patients?
It is now a mandatory requirement for all clinicians at the Trust that issue fit notes to patients in secondary care to complete the elfh Fit Note training module. It is designed to support a return-to-work conversation and ensure that you are aware of the protocols.
Will I receive a reminder to complete the training?
It is your responsibility to ensure that you complete the training in good time. There will be a regular communication up until the deadline, however you will not receive an individual reminder.
Will this training appear in ESR?
No, you will need to access directly via link provided. If you have not previously registered at elfh you will need to do so.
